Form 1095-C

Form 1095-C proves an employee either received or was offered health care by their employer. The form is sometimes provided to both the employee and IRS by the employer.

For 2016 tax filings, 1095-Cs are only required for employers that had an average of at least 50 full time equivalent employees in 2015.

If you had Gusto managed benefits during a tax filing year and met the requirements for needing a 1095-C, we will prepare one for you. If you do not have benefits through Gusto, please ask your insurance broker for assistance with form 1095-C.
