Evidence of Insurability (EOI)


Evidence of Insurability (EOI) is sometimes required by insurance carriers for life insurance enrollments. Here are a few examples of when the carrier may require the EOI:

  • If you select a coverage amount that exceeds the policy's Guaranteed Issue (the amount of money the policy pays out).

  • If you want to enroll in coverage after your initial new hire eligibility window.

  • If you’d like to increase your coverage mid-year.

The carriers will consider medical history while processing the enrollment application. After they review it, they’ll either approve or deny the application. Due to the sensitive information it contains, carriers will only communicate with you directly if they need additional information and reasons for declining the coverage.

If you have benefits with Gusto, we’ll remind you about your EOI requirements and will submit your EOI to the carrier if you provide it to us within 15 days after you completed your enrollment. After that, any requirements for the EOI will need to be handled between enrollee and the carrier directly.
